• Completed four (or more) consecutive workshops (required to be eligible for certification and a rewards package)
• Completed less than four consecutive workshops but will no longer facilitate (not eligible for certification)
• Send us a copy by mail, fax, or email – it must be signed by Host Group Contact
• Keep the original for your records.
