Workshop Tracking Form

EcoMentors use this form to track their completed workshops. This form verifies the workshop took place, what activities were used and provides any feedback from the Host Group.
Instructions:
- Complete the form every time you have:
  - Completed four (or more) consecutive workshops (required to be eligible for certification and a rewards package)
  - Completed less than four consecutive workshops but will no longer facilitate (not eligible for certification)
- Send us a copy by mail, fax, or email - it must be signed by Host Group Contact
- Keep the original for your records
